All students must complete an insurance mandated registration form prior to participation. Registration forms updated each September: 1) Read Studio Policies, below. 2) Print Registration Form, link button below. 3) Complete. 4) Submit to instructor. Please WRITE CLEARLY. Downloading/printing problems? Contact instructor. Studio Policies All Students: Registration: A signed registration form required for all participants/students. (See web site "Registration Form.") Student forms updated each September. Bring registration form to desired class with appropriate funds. Insurance fee prorated for new students. Questions--call the studio. Classes follow the Pendleton School District schedule. (No school--no A+PA studio lessons including holidays, weather, or emergency closures.) A $30 annual insurance/supply fee required for all students, except free community outreach programs. Dress Code: Please cover tummies, cleavage, shoulders, low backs, underwear, and mid-thighs. Dance classes wear the class attire. (See web site "Class Description/Attire" for details.) Also, no metal protrusions, belts, rivets, limited jewelry, etc. to protect mats and prevent injuries. Conduct code integrity, punctuality, dependability, follow directions; no drugs, alcohol, smoking, profanity or crudeness on the premises (students, parents and guests.) Students welcome 10 minutes before and after class, a fee may result for students earlier/later, (verified emergencies or prior arrangements with studio excepted.) Students and guests may not use any studio equipment without teacher permission and supervision. Payments: Due the FIRST lesson of each month, exceptions must have studio approval and may be subject to a $5 late fee. Make checks payable to A+ Performing Arts, Inc. (A+PA) unless otherwise instructed by your teacher. On checks, include student name and itemize payment (class/costume/music/registration.) If cash, please place in an envelope and include an itemized note. All payments must be accompanied with student name and itemized information to ensure proper credit. Payments are given to your instructor, unless otherwise directed. Semesters: Fall Sept.-Nov.; Winter Dec-Feb and Spring rehearsals/performances; Spring Mar-May; Summer June-Aug. General info: Parents/guardians/visitors welcome in all classes, provided guests do not disturb. (Dance class visitors must stay on the mats with street shoes. Noisy guests may observe through the dance studio observation windows.) In the event of an emergency, all attempts will be made to contact the parent/guardian, but the studio reserves the right to suspend instruction and seek appropriate emergency care, as necessary. Media: Students of A+ Performing Arts, Inc. may be included in media, including but not limited to, written media, photos, and video. Parents wishing to exclude students from media inclusion must notify A+ Performing Arts, Inc. in writing with each registration form. Music Students We provide notebooks and most music. Weekly written progress reports. Students may participate in the Oregon Music Teacher Association Adjudication Levels I-X (music theory, sight reading, rhythm, scales, and performance in Baroque, Classical, Romantic, and Contemporary music.) Each student receives trophies and medals as they progress, and, if desired, may compete at regional/state/national competitions. (Monthly music/recital fee includes all OMTA events and recitals except syllabus.) ALL students completing Level X receive a medal and a State Honors Recital invitation. Private music students practice requirements: Levels 1-2, 20 min/5x/wk; levels 3-6, 30 min/5x/wk; and levels 7-10, 45 min+/5x/week. Dance/Theater Students Students are encouraged to attend class regularly, but REQUIRED to attend the dress rehearsals AND performances, unless released by the instructor in advance (true emergencies excepted.) All dates are posted, in advance, on the web site studio calendar. Most classes have a costume rental fee and refundable costume deposit, rather than expensive costume purchase. (See "Costume Info" web page.) Students are enrolled by semester and are responsible for payments, costumes, and attendance for the entire semester. New students may attend 1 class without obligation, and may begin mid-semester with teacher permission. |
